Practice Charter
We do have a practice charter which can be viewed in the waiting areas. A copy can be obtained from reception.
Access To Medical Records
Patients may access their medical records by making a written request to the practice manager. A signed patient consent is required and there may be a charge of between £10 and £50
for copies of medical records.
Computer
All our patient records are kept on computer and the practice complies with the Data Protection Act. Anonymous information only may be used for audit, administration, research and verification checks by Coventry Primary
Care Trust.
Confidentiality
The practice has strict rules about confidentiality, which applies to all staff. Data may be shared with other healthcare professionals involved in providing medical care to patients. This is in accordance with the Data
Protection Act.
Consultations
Patients aged 16 and over can request a consultation if they have not attended a clinic within the last three years.
Patients over 75 years can request a consultation if they have not been seen within the last 12 months.
Change Of Address
If you change your address or telephone number, please inform the surgery as soon as possible.
Violent/Abusive Patients
Patients who are violent or abusive towards a doctor, member of staff or other patients on the practice premises will be removed from the practice list. |
