How Do I...
Register?

To register please contact the office manager.
All new patients are asked to see a practice nurse to register and have a health check. This enables us to obtain details of your past medical problems and to request your medical records from your previous GP. On your first consultation with a GP, ask for a 10 minute appointment.

How Do I...
Make An Appointment?

Consultations are BY APPOINTMENT ONLY but cases that are medically urgent will always be seen on the same day. To make an appointment please contact reception either personally or by telephoning 76592201. If you are unable to keep an appointment please let us know as soon as possible, so that we can use it for someone else.

You may see any doctor at the practice; please specify which doctor you wish to see when booking your appointment.

If you cannot keep your appointment, please let us know so that someone else can be fitted in. One appointment is for one person.

How Do I...
Obtain Home Visits?

If you are too ill to attend surgery, please telephone, before 10.15am if possible, to arrange a home visit.

Please give the office manager as much information as possible to enable the doctor to allocate priority to house calls.

Whenever possible we ask that you come to the surgery, not only because home visits are time consuming, but also because we have better facilities for examining and treating you in the surgery.

The request for a home visit is based on medical need - NOT for patients who have transport problems.

Emergencies Out Of Hours

If you require urgent medical advice when the surgery is closed, telephone the usual surgery number 024 7659 2201. Your call will automatically be transferred to Coventry Primary Care Trust Out of Hours Service who deal with all calls from 6.00pm to 8.00am on weekdays and weekends, also during bank holidays. Your conversation with the staff will be recorded.

Coventry Out of Hours Service is located on the Coventry & Warwickshire Hospital site, Stoney Stanton Road, Coventry. Car parking is available at the clinic.
Please consider whether your problem really needs a doctor urgently or if it could wait until the surgery is next open.

How Do I...
Obtain A Repeat Prescription?

This telephone line is purely for repeat prescription requests.

Please give 48 hours' notice for your repeat prescription Monday to Friday.

To Order

  • You may write.
  • You may telephone on Coventry 7659 2107 Monday to Friday between 8.00am - 12 noon.
  • You may call at the surgery during normal surgery hours Monday to Friday.
  • You can deliver the right-hand side of your prescription, indicating the items required, through our letterbox at any time.

Do not use the normal surgery telephone line for repeat prescriptions. Please leave it free for emergencies and appointments.

Collection

  • If you want us to post your prescription, please leave a stamped, addressed envelope.
  • Do remember to allow time for the post.
  • Prescriptions may be collected from the surgery either in person, by a responsible representative or by a chemist (if arrangements have been made with the patient).

The doctor will review your medication with you on a regular basis and an appointment will be needed to do this.

How Do I...
Obtain Tests And Results?

If the doctor has requested that you have a blood or urine test or an x-ray at the hospital, please ring the surgery after seven days for the results.

The receptionist will be able to deal with your enquiries after 11.00am or 3.00pm.

Freedom Of Information – Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.
This scheme is available from reception.
